Form Builder

The Form Builder module in SOURems allows you to easily create customized forms for event registrations, feedback collection, surveys, and more — all within a few clicks. You can design, configure, share, and analyze responses directly from your dashboard.

Creating a New Form

Click the Create Form button at the top of the form list to open the Create New Form wizard. A General Form has 3 steps; a Ticket Registration Form has 5 steps.

Step 1 — Category

Pick one of two categories:

  • General Form — Fully customizable. Ideal for surveys, feedback, and questionnaires.
  • Ticket Registration Form — Pre-configured with the Holder Name, Email, and Contact Number fields (locked). How submissions are handled depends on the behavior you pick in Step 2.

Step 2 (Registration only) — Behavior

Ticket Registration forms ask you to pick how registrations are handled:

Behavior How registrations flow
Standard Registration Attendees register and are immediately added to the ticketing system. Best for open events with no vetting required.
Approval-Based Registration Registrations go through an approval workflow. Once approved by the organizer, tickets are delivered through an email campaign sent by the organizer.
Paid Registration Registrations include integrated payment. Tickets are reserved at submission and finalized once payment completes. Use for events with paid tickets or tiered pricing.

For Standard Registration only, a Send ticket email on submit checkbox appears below the cards. When enabled, a required Support Email field appears — the address shown in the ticket emails sent to registrants.

Note: Behavior cannot be changed after creation — choose carefully. The chosen behavior is shown read-only on the Form Settings dialog later.

Step 3 (Registration only) — Goers or Pricing

This step changes based on the behavior chosen in Step 2:

  • Standard / Approval-Based → a Configure Goers step. Toggle Allow Goers to let registrants add extra attendees, and (Standard only) set Max additional attendees per submission and Max participants. Use 0 for no limit.
  • Paid Registration → a Configure Pricing step. See Paid Registration below.

Step 2 (General) / Step 4 (Registration) — Template

The wizard offers six quick-start templates filtered by category. Each card shows the estimated time to complete and the fields it pre-fills.

  • General templates: Feedback Form, Survey / Questionnaire, Event Feedback (each ≈2 minutes, 7 fields).
  • Registration templates: Basic Registration (1 min), Registration with Preferences (3 min), Registration with Session Selection (4 min).

Step 3 (General) / Step 5 (Registration) — Customize

  • Form Title (required) — auto-fills with the template's suggested title.
  • Form Description — auto-fills from the template.
  • A live preview card on the right shows the form as you type.

Click Create Form at the bottom to finish. The button shows "Creating…" while saving, then takes you to the Builder canvas.

Configuring Your Form

After creation you land on the Builder canvas (one of three tabs at the top: Builder, Preview, Responses).

Form Details

  • Form Title — display title shown to attendees.
  • Description — short purpose statement.
  • Additional Information — rich-text content via the SOURems WYSIWYG editor (supports bold, lists, tables, embedded images, etc.).

Adding Form Fields

The right-hand panel contains a Form Fields section above a palette of field types. Click any field type in the palette to add it to your form — the new field appears in the Form Fields section above the palette (not in the left panel; the left panel is the form list / navigator).

Field Palette

  • Preset Fields (5): Full Name, Company Name, Email Address, Phone Number, Country.
  • Other Fields (13): Text Input, Long Text, Number, Dropdown, Radio Buttons, Checkboxes, Date Picker, Time, Star Rating, Linear Scale, Choice Grid, File Upload, Terms & Conditions.

Drag the handle on the left of any field row to reorder. Duplicate field names are flagged automatically.

Editing a Field (gear icon)

Click the gear icon next to a field to open the Edit Field Settings modal. Common settings:

  • Field Type — switch the field's type. Locked for fixed registration fields (a yellow lock icon appears).
  • Field Label — the question or field name shown to respondents.
  • Placeholder Text — example text inside the input.
  • Field Description / Help Text — shown below the input. TinyMCE editor for Terms & Conditions; plain textarea for everything else.
  • This field is required — make it mandatory before submission. Locked on for fixed registration fields (except Company Name).
  • Only allow email that is valid in the current event — appears for Email-type fields. Restricts submissions to addresses already in your event's ticket / contact list.

Click Save (top right) before navigating away — an unsaved-changes warning appears if you try to leave the page with edits in flight.

Form Settings

Click the Settings button at the top of the Builder canvas to open the Form Settings dialog.

Behavior & Submissions

  • Submit Button Text — customize the submit button label (default "Share Feedback").
  • Success Message — shown to respondents after submission (default "Thank you for your feedback!").
  • Accepting Submissions — toggle this off to close the form to new submissions without deleting it. When off, the public form rejects new submissions. Useful at the end of an event when you no longer want responses but still want to access the data.

Form Card Appearance

  • Show Border — adds a border around the form card.
  • Show Shadow — adds a drop shadow for visual emphasis.

Registration Form Behavior (Registration forms only)

  • The dialog displays the chosen behavior type (Standard / Approval-Based / Paid) read-only with its icon and description. Behavior cannot be changed after creation.
  • Standard Registration — a Send ticket email on submit toggle is shown; when on, a required Support Email field appears (must be a valid email).
  • Paid Registration — the dialog includes a required Support Email, an optional Redirect URL after payment, a Ticket Pricing section (edit ticket types, prices, and stock — each row shows live Sold / Held / Available counts), and a Max tickets per order field. See Paid Registration.

The Save button is disabled if validation errors exist.

Form Template

Apply a branded landing-page template to wrap your form in a designed background. Click the Template button (next to Share) at the top of the Builder canvas to open the template settings popover.

Use Template toggle

The popover starts with the Use Template toggle turned off (state shown as Disabled). When you turn it on, a Select Template gallery appears below.

Available Templates

Template Description
Purple Gradient Modern split layout with a purple theme.
Teal Ocean Vibrant teal theme with floating elements.
Coral Sunset Warm coral theme with elegant waves.
Tip: Whether Use Template is on or off changes the public URL of the form. With template on, the URL points to the branded landing page wrapper; with it off, the URL serves the form on a clean minimal page. The Share dialog displays the right URL for your current setting.

Sharing Your Form

Click the Share button at the top of the Builder canvas to open the share dialog with three tabs.

Share Link tab

  • Copies a direct URL to your form.
  • The URL depends on your Use Template setting:
    • Use Template = off → the clean minimal form page (under /form/only/<id>).
    • Use Template = on → the branded landing-page wrapper (under /form/<id>).
  • The dialog shows a contextual note: "You can change this using the Template button next to the Share button."

Embed Code tab

  • Copies a full HTML snippet you can paste into another website.
  • Includes an <iframe> with a sensible minimum height plus a tiny JavaScript helper that listens for the form's resize messages and adjusts the iframe height as the form grows or shrinks — so the embedded form doesn't end up scrolling inside a tiny window.
  • A "Powered by SOURems" attribution link is included at the bottom.

QR Code tab

  • Renders a 300px QR code with high error correction.
  • Click Download QR Code to save it as a PNG. The filename is auto-generated from your form title.

Previewing Your Form

The Preview tab (next to Builder and Responses) renders your form exactly as respondents will see it.

Viewport toggles

Three buttons at the top of the Preview tab let you check responsiveness:

  • Desktop — full-width view.
  • Tablet — medium width to validate the mid-size layout.
  • Mobile — phone-width view.

Viewing Responses

The Responses tab shows incoming form data. Behavior depends on the form type:

  • Ticket Registration Forms — responses route automatically to the Approval Model for review (Approval-Based) or directly create tickets (Standard / Paid).
  • General Forms — responses appear in the Responses tab with two sub-tabs: Summary and Response Detail.

Summary sub-tab

  • Top of the page shows four stat cards: Total Responses, Total Fields, Refresh Data (clickable, reloads the latest), Export Data (downloads all responses as CSV).
  • Each form field gets its own card with a type-specific visualization:
    • Text-based fields (text, long text, email, name): a list of answers. For 50+ responses, the list virtualizes for performance. Each answer is clickable — click one to jump to its full row in the Response Detail sub-tab.
    • Choice fields (radio, dropdown, checkboxes): pie chart and bar chart visualizations.
    • Choice Grid: a grid heat-map visualization.

Response Detail sub-tab

Lists every response as an individual row, showing the answers to every field for that respondent. Use this when you need to read someone's full submission.
